Service Coordination for Children & Youth

This service is designed to help families with children who have highly complex needs that cannot be met within the existing service system. Upon referral and through consultation, a plan will be developed to meet these extraordinary needs.  The Coordinator will meet with you to review all the interventions and services that you have tried to date.  She may, with your permission, organize a ‘case conference’ with all appropriate service providers to create a coordinated approach.  Sometimes short-term ‘hard to serve’ funding is sought to bridge to a more permanent plan.
