Guide for Caregivers

Intermediate Years it's time to:

Age 11/12

  • Start a transition plan to high school, including a Grade 8 completion date

Age 13/14

  • Find out when there is a transition information session at the high school. Begin to develop a transition plan with the school for my son/daughter (Individual Education Plan (IEP))
  • Contact the special education resource teacher at my son/daughter/s school to arrange an appointment to discuss the upcoming transition with the special education department head at the high school

High School Years it's time to:

Age 15/16

  • Establish a high school program completion date in consultation with school staff and ensure it appears on my child’s Individual Education Plan (IEP)
  • Contact one of the agencies mentioned under Additional Considerations (if not done previously)
  • Develop a Transition Plan for my child with school staff and other service providers if appropriate (i.e. Developmental Services Ontario (DSO), Point in Time, Community Living)

Age 17

  • Work with the school to review and update the Transition Plan for my son/daughter
  • Make sure my son/daughter has a Social Insurance Number (SIN) and an individual bank account
  • Contact one of the agencies mentioned under Additional Considerations (if not done previously)
  • Complete the intake with Developmental Services Ontario (1-855-277-2121)
  • Complete the application form for Ontario Disability Support Program (ODSP)

  • Investigate possible post-secondary education options, such as Community Integration through Co-operative Education (CICE) programs at community colleges.

Age 18

  • Work with the school to review and update the Transition Plan for my son/daughter
  • Contact one of the agencies mentioned under Additional Considerations (if not done previously)
  • Update Developmental Services Ontario as to your child’s support needs and service requirements

Age 19

  • Review and update the Transition Plan for my son/daughter.
  • Finalize post-education arrangements including residence, day programs, and/or post secondary education
  • Ensure that arrangements are made for continued access to essential equipment after leaving high school

Additional Considerations:

  • Identify a person to assist me and provide support in advocating and applying for adult supports for my son/daughter
  • Apply for a Social Insurance Number (SIN) for my son/daughter
  • Establish a bank account for my son/daughter that can be used throughout high school and post-secondary
  • Encourage my child’s involvement in community activities such as cadets, sports, dance, and music
